Notice to users:  Usage of this form is designed to speed up the check in process by providing changes to the tournament director in advance. (7 days or more days is preferred.) You are still required to check in and confirm these changes.

Please remember to bring your USBC membership cards and any supporting documentation such as average verifications forms, league sheets, etc,.  

These changes are forwarded  to the tournament manager via E-Mail.  Computers and the internet may appear to operate instantaneously, however the individuals downloading the information, need additional time.  Please be patient .
